Population Health Management Transitional Care Services Summit

To provide technical assistance and facilitate collaborations and shared learnings as MCPs implement TCS for all members, DHCS will convene a one day onsite ‘PHM Transitional Care Services Summit’ on Wednesday, March 6 in Los Angeles, hosted by LA Care Health Plan. The summit will include MCP leaders and staff (e.g., CMOs or program lead), as well as select hospital, SNF, and primary care provider participants who are leading the implementation of TCS at their organizations.
Context for the Summit:
Over the first year of implementation for the CalAIM Population Health Management Initiative, a primary goal for DHCS, in partnership with MCPs and other stakeholders has been to ensure members across the state are effectively supported during transitions of care. Transitional Care Services (TCS) requirements have been live for high-risk members since 1/1/2023 and the next phase of implementation for all members has just launched on 1/1/2024. Based on stakeholder feedback, DHCS recently released an updated TCS policy beginning 1/1/2024.

The Summit is a closed-door and limited registration event. Invitations are not transferable. Each MCP is invited to bring around two attendees in-person. We highly encourage onsite participation if possible. A virtual participation option may be available but is not guaranteed.
Topics Covered:
Attendees will hear from DHCS program leads about policy refinements and key priorities that the Department will focus on for the second year of implementation and beyond. There will be panel discussions and group breakout sessions regarding key pillars of TCS, such as discharge collaborations and primary care follow-ups. Participants will have an opportunity to learn from each other, share best practices, and prepare to advance key DHCS TCS priorities in 2024. A detailed agenda for the day is forthcoming.
